Are you based in a clinic or do you provide home-based therapy?

Soar Pediatric Physical Therapy can provide services in our clinic located in San Rafael. We share an office with both an occupational therapist and a speech therapist if other needs arise. We can provide home-based and school/daycare-based as appropriate.

Do you work with Golden Gate Regional Center?

Yes. I have been vendorized to provide physical therapy services to children 0-3 years of age through Golden Gate Regional Center (GGRC).

Are services available in a language other than English?

Yes. Carrie speaks Spanish conversationally.

Do you accept insurance?

I am contracted with Meritage Medical Network as an “in-network provider”. Otherwise, I am happy to provide you with a superbill that you may submit to your insurance carrier. I do not bill insurance companies directly. Payment is due at the time of service.

You may also work with your pediatrician’s office to receive a “gap exception”. A gap exception in insurance allows coverage for out-of-network services at in-network rates when there are no in-network providers available to offer the needed care.

Do you offer telehealth consultations?

Yes. Telehealth services are available upon request.
